Key Care Membership

In an effort to better serve our patients, we are now offering the Key Care Membership for patients who wish to continue their membership with the Clinic, but also require less frequent appointments. This membership is open to patients who have had at least one year of standard membership and is offered at the discretion of the medical team.

Membership Details:

Appointment Availability:

Four (4) physician visits per year with an additional two (2) visits allocated for urgent or transitional visits. Should more than six (6) visits be required or desired, a standard membership can be reinstated. Nursing appointments are included with the membership fee and will only accrue a charge should a visit be missed. Medication management throughout the year is also managed at no additional cost.

Fees:

$2,000 annual membership fee with follow-up appointment fees equivalent to those of the standard membership:

Dr. Lischke: $200/25 min. session
Dr. Das: $200/25 min. session
Dr. Robbins: $200/25 min. session

The annual membership fee will remain a non-refundable fee. If a patient is referred to or wants to access unlimited visits at any time during their Key Care Membership (i.e., a return to a standard membership), the balance of the remaining months of membership will be prorated towards the cost of the full membership for the remainder of the year. A renewal session is not included in the Key Care Membership.

Criteria:

Patients must be a current or past member of the clinic; this membership is not open to new patients. For current patients, their physician and nurse must refer them to the Key Care Membership. For past patients, a one hour paid visit (not to be included in the Key Care Membership limit) to evaluate appropriateness might be recommended by the medical team.
